The personal learning network for educators

Our first quarter is coming to a close and I would like to create a survey to get feedback from my students about a variety of topics.  However, I admit to being intimidated by what I might be told.  Ideally, I would like to develop questions that reflect on the strategies, assignments, expectations, etc. rather than on personal feelings towards me. 

Does anyone have advice on how I can do this?  What is the best way to create non-biased questions?

Any advice or directions to take online would be greatly appreciated!
